Integrated Care: Identifying Individuals at Risk of Becoming Complex Patients
This competition theme, led by South West AHSN, focused on the need to identify and understand the cohort of the population that would benefit from enhanced integrated management of their long-term condition(s). The purpose of identifying this group was to ensure care was person-centred with the aim of preventing avoidable escalation of complexity and care need.
