Brave AI is a clinical decision support tool for prediction of all-cause unplanned hospital admissions over a 12-month period using only Primary Care records. Brave interfaces with routinely-collected patient information and provides patient-level predictions at a Primary Care Network (PCN) level. The insights produced by Brave can be used by proactive teams of healthcare professionals to deliver personalised care planning in our communities, enable self-management at home, and anticipate exacerbations of chronic conditions. |
