In order to drive sustainable wellness and long-term condition management, patients need to be at the centre of an integrated care pathway, empowered with appropriate access and incentives to manage their own care. This project will assess the feasibility of implementing personal healthcare records, care provider communications and approval processes on to a private Blockchain. Current processes and data flows will be reviewed, improved and converted to Blockchain solution standards. A working prototype will be created and tested to evidence technical feasibility. A short and long term cost benefit analysis will be undertaken, and ability to deliver at scale assessed.
