Momenta’s market-leading cardiovascular disease prevention programme (CPP) has been co-developed with South Eastern Trust in Northern Ireland with input from British Heart Foundation.
This project is a multi-partner, cross-sector demonstration project to optimise scalability across three care-pathways in Birmingham and Solihull. We will develop a promising programme into a fully-supported service and recruit sufficient primary and secondary care referrals and self-referrals to start 700 patients on the 9-month, 18-session group programme, delivered virtually and in-person.
This project will lay the foundations for rapid scale-up to support a large and high-need patient group that currently lacks replicable and scalable behavioural interventions.
