| Project | Improving cardiovascular disease outcomes in primary care |
|---|---|
| Description |
Docobo’s Aegle system, developed in partnership with Crawley, Horsham and West Sussex CCGs, will support proactive programmes in primary care focused on CVD. Aegle integrates ‘pointing’ to patients at risk from existing risk stratification with remote monitoring technology to deliver individual care plans and track patients through screening, assessment and monitoring. It will enable clinicians to manage CVD as a single family and optimise a case finding approach that relies on the specific cardiovascular conditions. |

| Project | The OptNIVent telehealth system |
|---|---|
| Description |
Developing in association with Aintree University Hospital, OptNIVent, to optimise non-invasive ventilation during palliation. Motor Neurone Disease (MND) is a progressive disease that attacks the nervous system where messages gradually stop reaching muscles including those which help us breathe, often resulting in the need of a ventilator for the last 18-24 months of life. The OptNIVent telehealth system enables more frequent ventilation management, down from once every 3-4 months to once per week if necessary and without increasing the number of clinicians involved. The improvement means less anxiety for patients and the regular health checks, recorded on the system, help clinicians to intervene earlier to prevent illnesses from developing. |
