The Lenus COPD Support Service was co-designed with clinicians and patients to digitally transform the long-term management of Chronic Obstructive Pulmonary Disease (COPD). Reducing serious disease exacerbations is a priority for patients’ wellbeing and providers, who are faced with unprecedented demands in unscheduled care including rising COPD-related emergency admissions. The Lenus COPD Support Service provides an innovative and sustainable digital solution to support proactive COPD intervention outside of the hospital with a greater focus on pulmonary rehabilitation and community supported self-management. https://lenushealth.com/
