A collaborative project led by uMotif, with HERE, Care Unbound and the Picker Institute to design, deploy and test a simple, effective and low cost digital platform for primary care to capture key outcomes, experience and symptom data to help patients in self-care, GP practices support patients safely, and Federations / Commissioners to understand care.
