With partners at Crawley, Horsham and Mid Sussex CCGs, Docobo is developing an analytical intelligence system for targeted population health and well-being. It will provide rich intelligence to identify people with complex needs, including those with a particular risk of social isolation and support care co-ordination between agencies leading to improved services in the community. |
