Project The Digital Sleep Diary: The self-management and remote monitoring app
Description

Our project proposes a pioneering approach to support self-management and clinical decision making in primary care, aligned to better utilisation of limited resources in secondary care. Building upon our proven technology used in another long term condition we would:
I. Create a downloaded app designed to take the young person on a journey that will either help resolve their sleep challenge, where this is linked to 'life style', or point to more challenging sleep disorders (and evidence this) where specialist intervention is required.
II. A user-interface co-designed with patients and users with the right levels of 'nudge' and 'nurturing' to achieve optimum interaction.
III. A clinical web portal accessed through any browser, on any computer, protected with password security. The portal holds the patient record and enables clinicians to have a graphical representation of their patients.
IV. An algorithm (hosted in the cloud and accessed through the secure clinical web portal) that undertakes all of the calculations and generates an indicative diagnosis.

Project Adjustments by Open-OH: Work Related Mental Health Interventions for People with Long Term Conditions
Description

Adjustments by Open-OH is a digital tool that supports workplace mental health for people with long-term conditions such as Fibromyalgia, Long Covid, ME/CFS, and POTS. It helps users initiate, evaluate, and refine workplace adjustments with employers, reducing barriers and promoting well-being. Building on previous Open-OH research, this project enhances the platform with co-designed, evidence-based interventions to address mental stressors such as anxiety, brain fog, and stigma. Users can track health data, access self-management resources, and find specialist support. Through real-world testing and evaluation, the project aims to improve workplace inclusivity, minimise symptom exacerbation, and support sustainable employment.
