Project The KYOH wellbeing snapshot and integration toolkit
Description

The Know Your Own Health (KYOH) Wellbeing Snapshot & Integration Toolkit enables GP surgeries to offer personalised supported self-care in their practice as a step towards delivering integrated care.
Supporting people to self-care can improve people’s quality of life, clinical outcomes and positively change their patterns of contact and behaviour with health and care services. It can also reduce unnecessary hospital admissions, result in fewer visits to the GP surgery and the risk of increasingly complex comorbidities.

Project Identifying individuals at risk of becoming complex patients
Description

We Predict offer predictive analytics as a service to the health sector. The company uses epidemiology and statistics to analyse data and predict outcomes to improve patient experience, planning and health. The early stages of the project focusing on predicting patient complexity and the likelihood of unplanned admission to hospital within the next 12 months for particular population cohorts.
