Project Social AI empowering families for elderly support in the home
Description

Upstream believes that prevention is better than cure. Its mission is to facilitate the shift to earlier preventative care delivery and support people to stay well for longer in the places they want to be.

Upstream has developed a solution called Bridgit™. Bridgit provides a unique ecosystem of products that support patients to take the actions needed to keep themselves well, reassure family members that all is OK and enable stretched care teams to provide focused earlier interventions. Bridgit uses Microsoft’s AI capability and cloud services, along with new hardware devices (home hub and watch) specifically designed for accessibility by an elderly population. A population who may not have access to the internet or know how to use mobile applications.

Project Integrated self-management and prevention for multiple musculoskeletal conditions and pathways
Description

We provide self-management and prevention technology for musculoskeletal (MSK) conditions and Occupational Health.  Organisations integrate getUBetter so that patients can be linked to their local self-management pathways via clinicians, support staff or independently from GP websites 24/7. 

Health Organisations standardise their care, provide bespoke local content, automate referrals, promote self-management, stratify care and reduce costs.  Patients receive personalised and targeted self-management and prevention whilst being automatically connected to their local healthcare provider, treatment and support services. 

getUBetter provides access to multiple MSK conditions from a single app, enabling standardised care to be configured to multiple organisations simultaneously and “on-board” new conditions cost effectively. 

We are currently developing new technology, simplifying integration, improving behaviour change, evaluating and optimising our scalability.
