Project Scaling a digital support system to reduce workforce pressure
Description

Brain in Hand (BIH) is a professional digital support system designed for use by health and social care providers. The system provides a unique combination of digital technology and professional support enabling users to access help whenever and wherever it’s needed. Users are able to manage stress and anxiety through user-led interaction with the system. Early intervention prevents the escalation of a user’s condition which in turn prevents more costly service provision further down the line.

Evidence from multiple case studies across 20% of English Local Authorities and across 4,500 users demonstrates that BiH is making a major contribution to supporting self-management, helping users achieve their goals for independence and reduce demand from health and social care provider teams.

Project The Tribe Project
Description

The Tribe Project is a co-production driven, social action initiative to upskill members of the community within areas of care inequality to become micro-commissionable community carers.

Through combining the latest advances in machine learning and smart cities technologies, Tribe geospatially maps patterns in community ‘need’ ultimately predicting where service demand will intersect with areas where care provision is failing or none existent.

Working within these areas Tribe supports the digital upskilling of voluntary organisations, community groups and people seeking to start their own care micro-enterprises. Tribe directly results in skilled employment for local people in areas of care inequality.

Project WellwisePro: Improving care through AI powered insights into health and activity
Description

Wellwise Pro will:
Provide pro-active, relevant care based on a broad range of areas.
Defined gateways enabled indicating decline or improvements facilitating early intervention.
Prevention - Unique user normal behaviour defined with key variations flagged. Anonymised data overlaid to those in similar groups to help early identification / prevention
Enable carers to have much greater visibility on a day to day basis how users are and to prioritise those in need whilst enabling users / family members / local support carers to be more proactively involved in care
