Project MyBrainBook and NHS 111 integration: Supporting personalised care pathways in dementia
Description

MyBrainBook is a digital platform that allows patients with cognitive impairment and dementia to capture information about themselves and share it with friends, family and health and care professionals. The personalised care and support plans include an ‘urgent care plan’ which, building on integration with London NHS111 services, gives the emergency and out-of-hours services real-time access to information to help ensure the appropriate pathway is chosen, avoiding A&E admissions where possible. For this to be fully realised there is a need to ensure that staff are trained and there is awareness amongst care providers and the public of alternative pathways.

Project TACTIC – Team Communications for Integrated Care
Description

Careflow is proposing to develop its messaging and care coordination platform to support innovative community-based schemes that aim to reduce pressures on A&E admissions by enabling self-care for patients with Long Term Conditions (LTCs).

By using Careflow, multi-disciplinary teams working across multiple organisations can communicate securely in real-time by web and mobile about a patient, receive automatic patient events from emerging self-care application and clinical analytics tools as well as legacy hospital administration systems to make prompt care decisions. This helps care management and transfer of care between care team members.

Project Traumatic Brain Injury (TBI) - Point-of-Care Diagnostics Test
Description

Biovici proposes to develop an affordable, portable, point of care test to detect the presence of biomarkers linked with Traumatic Brain Injury (TBI). The device will identify biomarkers present in saliva, so as to create a non-invasive test, but also will be developed to analyse blood from a pin prick. This test can: potentially reduce the number of people attending A&E in the first place through screening at point of incident, aid triage and effectively manage the flow of patients into the correct departments, be utilised to aid the clinical decision maker by providing objective feedback and help reduce misdiagnosis
