Project Making Young People’s Asthma Self-management Smarter (MYPASS)
Description

The Digital Health Passport is an asthma self-management app co-designed with young people and NHS specialists; aligned to clinical guidelines (NICE, BTS/SIGN, GINA) to improve asthma control. MYPASS will measure the impact of the platform on asthma attacks, unscheduled care and steroid use and evaluate three user engagement strategies (Nurse-led, Social Media and NHS Comms).

MYPASS will deliver:
Smarter asthma plans with signposting from symptom trackers.
Improved medication adherence - repeat prescriptions, medication reminders, inhaler videos.
Trigger avoidance - air quality alerts, allergy education.
Knowledge - seven core modules with regular reinforcement.
Improved patient activation - skills, knowledge and confidence to self-manage.

patientMpower

Project A randomised, prospective evaluation of health outcomes, service and health economic impact of hybrid (remote+clinic) versus clinic alone to enable right-time, right-place care for the follow-up of patients after lung transplantation
Description

patientMpower is a specialist digital healthcare solution for chronic respiratory conditions, combining home monitoring of objective and subjective measures with data insights and patient-tailored alerting features. This project will evaluate the health, service and health economic outcomes of digitally-enabled hybrid care, versus standard care, for patient follow-up after lung transplantation.

PocDoc

Project Improving primary and secondary prevention of CVD and keeping people out of hospital through easier, more cost-effective access to lipid testing
Description

PocDoc has developed a world-first smartphone-based lipid test which can deliver a 5 marker lipid panel, via the PocDoc app, within 6 minutes, with results shared immediately back with the healthcare system. This allows testing for lipids to move out of GP surgeries, dramatically increasing access to testing which will in turn prevent more people from developing Cardiovascular Disease.
